I also authorize the publication of any positive comments and/or feedback to be used on those platforms and the business website. By signing the form you agree to have your name and/or child's name to be used in conjunction with comments provided by you via the website or private messaging service in promotions and posts on social media. I agree to make no monetary or other claim against Healthy Sprouts and House Calls for the use of these images or videos for the intended purpose. I understand that this authorization is voluntary. I may refuse to sign this and in doing so no photographs or comments will be shared on any platform. My refusal to sign this form will not affect my ability to obtain medical treatment for my child through Healthy Sprouts and House Calls, PLLC or receive insurance payment for any services rendered for health care purposes in the future. I declare that I have the authority to sign this form as a parent or guardian of my minor child in the event that content is used on social media or the business website.
